Service agents will be able to help answer questions about the Horizon Blue Cross Blue Shield of New Jersey program you may be eligible for. Please mention the name of your employer, so we can access information about the program(s) you may be eligible for right away.

If you are a Horizon Blue Cross Blue Shield of New Jersey member — If you are an enrolled Horizon Blue Cross Blue Shield of New Jersey member, contact us via our National Member Services Department at 1-800-355-2583. Please be sure to have your Member ID Number ready, so we can begin helping you right away.

Finding a participating BlueCard^® or BlueCard^® PPO provider — If you are an enrolled member or are thinking of enrolling, you can access information about participating BlueCard and BlueCard PPO doctors and hospitals anywhere in the nation via our BlueCard Access Line at 1-800-810-2583. If you are not yet an enrolled member, simply indicate you are not yet enrolled and researching information in order to make your enrollment decision.
